The goal of Integrated Care Management is to promote and maintain
independent living for
frail
elderly and adults with disabilities, thereby decreasing the use of institutionalized care. The program provides comprehensive, long-term services utilizing the care manager’s skills to link clients with a full range of appropriate services and available funding sources. Care management provides intake screening,
in-home assessment,
care planning, service payments,
case monitoring and periodic reassessment.
In addition to the
Area Agency on Aging, other entities contributing to this program are the County’s
Adult Protective Services program, the
Department of Public Social Services, the California Departments of Social Services and Aging, as well as a network of community-based provider agencies.
